Unable to add new patient

Open EMR was running fine yesterday, no problems.

Today, we cannot add a new patient, but get the error:

Object not found!

The requested URL was not found on this server. The link on the referring page seems to be wrong or outdated. Please inform the author of that page about the error.

Running OEMR v. 5.01 (6), on Windows 7 x64, Firefox Quantum V 64, x64.

We can edit existing demographic files and see the calendar, but cannot add a new patient.

Same problem on different client computers. I’ve restarted XAMPP and MySQL on the server, cleared the local Firefox caches, no help. The last questions on this issue date back to 2010-2012,

I tried https://localhost, and get “Unable to connect”

Where do I start to look?

I find a number of errors in the Apache log, but I cannot find a specific PHP error log.
I find the following logs:

xamp-control.log O:\xampp
access.log O:\xampp\apache\logs

These errors have occurred periodically in the O:\xampp\apache\logs\error.log since I installed V5.xxx last July:

I have increased the maximum execution time in php.ini to 120 seconds, and the maximum input variables to 6000, without benefit.

The changes that I made were to convert some of the Demographic LBF fields to Unused, and I changed one of the User Defined fields to an Age field. I did not delete any of the preconfigured Demographic fields. ( learned my lesson in the past).

I noted that the preview function in the LBF designer page shows only a blank page. I see the following error appear:

I can see no evidence that the directory …\DEM\ with the file “view.php” ever existed, even checking the backups back to last July. I suspect that would explain why the demographics preview fails to operate.

It looks like whatever I did broke the Demographics LBF. Which file or files would I copy back from a previous daily backup to return it to the function earlier in the week?

I set some of the superfluous predefined Demographics LBF fields to “Unused”, but did not delete any of them. I learned that the hard way a couple of years ago.

I changed the definition of the first user defined field to an “Age” field, but it doesn’t calculate an age from the date of birth Still have to work on that one.

Suggest recreating your error (ie. open a new patient demographics screen) and then checking to see if it caused any errors to get logged into O:\xampp\apache\logs\error.log .

Looks ok. Next is to look at what Jerry suggested, which is the browser console (for javascript errors).



Then recreate the error and see if any errors get logged.

I’m no java expert, but it looks to me like that’s just a benign status message that the error popup page has been displayed.

Additional Info:

If I click the “search” button instead, the search window comes up empty, even though there are at least 4 other patients with the “Imaginary” family name in the database. No error messages appear in the monitor window, just informational messages that the dialog window has been created, and then hidden and deleted. Seems like normal operation.

You want to look at the php error log not apache error log. O:\xampp\apache\logs\error.log

Shouldn’t be a problem as long as none of the id’s were changed. Is that the case? Also note if you are using the encounter preview feature, it will not display any of the core lbfs and is meant as an aid when creating new forms for encounters.

What is the name an location of the PHP error log?
I’ve looked at all the error logs in the xampp directory, but seen none by anything resembling that name.

I did redefine one of the user defined fields to hold allergies, but I didn’t think that would break anything.

I have put it back to the original condition as best I can, but that didn’t fix anything.

Is there one file, or a group of files that i can copy back from one of the earlier daily backups that would restore the Demographics LBF? I do daily backups with a total XCOPY of the complete xampp directory every day.

Let me look into the code a bit and will get back to 'ya.

@Wizard353 Can you give me exactly what fields you modified? I don’t think this should cause the issue but I need to be sure before moving on. My main concern is an ID was changed not necessarily the label but, one never knows…

I was in a hurry when I did it, but I’m pretty sure it was userlist1. I’ve put it back that way, but that didn’t fix anything.

Having hard time understanding what is going on here. One, when you did the console error logs test, did you actually try to add new patient? If you are getting a 404 it should show up in the console log. Would be nice to know what the request link that can’t be found is… Two, changing the user defined fields should be okay however if you changed UOR on some other fields that are required for validations then that may be an issue. I need something to go on. Maybe screenshots of Dem LBF or if you have to, a backup of your database.

It’s like the error level is turned off in browser console. Should have Errors highlighted in blue and then turn off Warnings on your browser console toolbar. There has to be a 404 error with the file info show up in the log if you’re getting a 404 error in the UI. With everything you have posted here I still don’t have a notion of how this error is occurring. I know what I expect to see though.
May be easier if you can give me a test login so I can recreate myself. You can PM or email me if you want to do that. Or a backup of database could help me recreate. I just don’t want to keep you hanging for office hour tomorrow if I can help.

Thanks for all your efforts.
I will roll the system back to Wednesday, and see if it works again. It was a very quiet week in the office, and only a handful of new patients were entered. I’m currently using OpenEMR only for demographics and E-prescribing. At least I won’t lose any patient encounter records.

I will be more careful about modifying the Demographics LBF in the future.
