X12 file log error

Hi,

We need help with the log errors after generating the X12 files.
We got " Supervising provider has invalid last name " in the log.

This happened in the fee sheet, if we put:
Providers: Rendering [provider A] Supervising [–N/A–]

or

Providers: Rendering [provider A] Supervising [provider A]

However, the log will not generate above error message if we put 2 different practitioners, such as:
Providers: Rendering [provider A] Supervising [provider B]

The treatment is performed by one provider so we don’t need the supervision option. It must indicate N/A!

My friend is using openEmr 5.0.0 without any log error like this. Is this the problem with v5.0.1 (3)? Could anyone please help out here?

We run openEMR on Windows 10.

Thanks! This problem is solved. The 1500 form has a problem still.
