This was a rather unstructured discussion, which covered a wide array of topics.

There was a lot of discussion on the Zend framework proposal by ZH Healthcare (even in the absence of Shameem) and the scope of MU2 criteria that they will be fulfilling. Very cool.

There was then a discussion on usability and mention that there is a MU2 criteria that focuses on this.

There were then some questions by a student, Carmen, in the ā€œThe Columbia Health Information Technology Certification Classā€ regarding how OpenEMR is organized and how the development and management process works, which lead to several descriptions of the OpenEMR community and a recommendation by Rod to read the ā€œThe Cathedral and the Bazaarā€:

(at this point I had to answer a page and missed 10-15 minutes of the call)

Then we discussed patient data importing mechanisms, of which there are several options in OpenEMR. David has one in Zend, and I pointed out the current CCR import mechanism that was recently contributed by ZH Healthcare. Virginia (Instructor of the Columbia Health HIT Certification class and expertise in HL7) then educated us with an overview of CCR, CCD and consolidated CDA formats and that MU2 requires consolidated CDA, which is used as a building block in several of the MU2 criteria.

Regarding Meaningful Use 2 progress, here is the updated barometer:

As previously, this was another rather unstructured discussion, which covered several topics.

Discussed the API that is currently being worked on and maintained by MedMasterPro. Gary is from NASA and is interested in creating an API and they plan to use the code from MedMasterPro’s API code as a starting point in their solution, which I agreed was very reasonable. Hopefully there will be time in future for a developer to continue the reviews of these API sets to get them into the OpenEMR official codebase.

Discussed that there appear to be many other smaller EMRs that are not getting MU2 certification (meaning they are likely closing shop). Reasons for this were thought to be related to technical hurdles of MU2 in addition to the competitive environment of EMRs.

There was an update on the progress in the Zend framework code for importing modules which is almost ready for an official code review. I pre-reviewed the code over the last week in order to hopefully get it ready for an official efficient review by the community. This code will serve as the launching point for several modules that ZH Healthcare plans to build to fulfill many of the more difficult to obtain MU2 items.

There was then a discussion regarding many of the forks out there which now exist. Art brought up an entertaining anecdote where he was asked to help out with another clinic’s EMR with some fancy name and website that he realized was OpenEMR renamed with some customizations. Turns out there are likely hundreds of these things out there.

Virginia announced that her Columbia HIT Certification Class is going to analyze the Patient List Creation MU2 item, which is very cool and look forward to seeing how it turns out.

Attendees: Jack, Julie (EMRConnect), 5 folks from Columbia U: David, Virgina, Angie, and my apologies (please correct name) Nina, and a CU alum (also nameless).

Obviously the discussion centered on the CU project. A couple of specifics were: 1) process and authorization for Snomed data import and 2)Under Past Hx, there is a paucity of predefined options and even at best these represent text entries. Discreet diagnosis data (diagnoses with standard codes(icd-n or snomed) via drop-down menu or search function) and discreet family relationship data for family history (first degree vs lesser relationship) would likely be needed for MU2 either as specified functionality or as a ā€œusabilityā€ feature. In any event, it would be useful for quality patient care. e.g one could then set a ā€œreminder ruleā€ for anyone with a first degree relative with colon cancer to begin colonoscopy screening at age 40.

Question about the table currently used for family history and if/how it would be modified to accommodate the above.

  4. Return to this page and you will be able to complete the Snomed installation process by clicking on the SNOMED section header

NOTE: Only the Biannual International Snomed Release is currently supported.

The following International Snomed Release languages are supported: English, Spanish (Additionally, the corresponding US Snomed Extension Package can be imported separately after installing the International (English version) Snomed Release)

WARNING: If you are using the 2012-10-31 Spanish version, then note you need to unzip it once before placing it in the contrib/snomed (this is because the released file was double-zipped).

Looking forward to the next meeting this Saturday. It’s almost 2014 and OpenEMR is still far away from MU2 certification…

The OEMR has a balance of $8,791.07.
We need a minimum of $25,000 to certify assuming ALL volunteer coding,
which is unlikely.

I think we need $100,000 realistically to better that just barely passing
the bar.

Exciting since the group has several developers and may be willing to contribute some funds towards MU2. Sounds like the first project will be improving the billing engine and placing it on par with the other billing software out there (ie. medisoft etc.).

Lou discussed his work which is very cool. His group essentially goes to physician practices which use non-OpenEMR EMRs that don’t support CQM calcolations and his group actually brings in OpenEMR temporarily to only calculate the clinical quality measures (since the EMRs that those physicians have do not support the CQMs). Lou, correct me if I am wrong here on the description.

There was also some discussion on frameworks and ongoing Zend contributions.
