Situation
I have recenltey installed a new open emr version 7.0.2 and I was not able to locate the vitails when adding a new encounter. also when creating the encounter i AM NOT ABLE TO see the other documents part

Check under “Admin → Forms → Forms Administration” whether “Vitals” Form is Enabled. If not Try to Enable that and Check.

Thanks for your responed, It is enabled I have disabled it and re-enable it and signed out and in but still not able to see it
Please check the error log for any errors and post here - it will be useful for community here to check and update on that.
When we upgraded from V5 to V7 we had an issue with the vitals section on certain patients.
The problem was that in the form_vitals table. Apparently in the past some of the patient’s height were entered incorrectly. Like 71 inches was entered in as 710. The big number caused some sort of a calculation error in V7. The result was there was no vitals info rendered.
By going into the DB and correcting the height values on the patients that had issues solved our problem.
You may have a different problem than this but maybe not. Anyways, if you are going to poke around in the DB always take precautions like backing up the DB before messing with or using a sandbox system for troubleshooting.
Found the issue, when I installed the the fresh EMR system the filesize was less than the usllay I had to resintall it again. some of the js file where missing.
