V5: Misc Billing Options for HCFA-1500 Box 15

I just upgrade from V4.2.2 to V5 and notice that Box 15 in the Misc Billing Options for HCFA-1500 no longer available. Is there a way to add that back?

This is how it looks in V4.2.2

hi @2017, try this replacement new.php (12.5 KB)

it replaces the following script openemr/interface/forms/misc_billing_options/new.php
