Hallo everyone. Kindly assist me here. I want to create a new open form called ‘paediatrics’ using the already existing ‘care_plan’ as a template. I got to openemr/interface/forms/ and copy ‘care_plan’, paste it in the same folder and rename it ‘paediatrics’. After this I edit all the php files and the table.sql accordingly to modify the form. Then I go to the Forms and Install the db. Up to here all this look ok!
However, after entering encounter data using the form and saving it, the name still remains ‘Care Plan’. Please help me to circumvent this issue!
