Updating Form templates

Hi! Is there a way to update the default form templates such as the referral form?

In terms of what I mean by updating, I would like to include my center’s logo and format it how I would like it to be rather than how it is by default. Is this possible to customize these template documents?

I have not edited the referal form, but have you looked atinterface/patient_file/transaction/print_referral.php
