Terminology for Issues

I guess everyone here knows that “issues” in OpenEMR are used to track medical problems, allergies, conditions, etc.  From time to time I have heard complaints about the choice of the word “issue” as not being common medical terminology.  It’s true that I pretty much just made it up.

One client of mine recently suggested calling them "cases" instead.  Anyone have an opinion about that, yes or no?

In ICPC (the coding system generally accepted in European setting for the Family DOctors/General physicians) we have the "Reason for encounter". What is similar to "what is the complaint when the patient is in search for medical assistance".

What happened in the ISSUES development is: "all relevant history and active complaints/ilnesses is packed together and is shown as P, M, A, S, D. This gives an oversight of the medical history and possible running complaints. So the thing to do is accept this as is and keep the name (Rod-)ISSUES or make it like something doctors are used to have, a seperate view for: Medical History, active Complaints, Allergy. Dental is usually only for Dental Clinics.

If you are working on this part, Please make Allergies more pronounced (stand-out). Give it a color on the screen like RED and/or BOLD. Too often patients get the wrong treatment due to oversight of the allergy noted on the chart.

or me personal you can keep ISSUES, since it is a broader entity than reason for encounter. Reason for encounter is the same as the "S" subjective in SOAP. Once used to ISSUES and knowing that it is a systematic compilation of the medical thing for a client I know where and waht to look for.

Don’t make it history since ISSUES is more than only History.

It could be a "Medical Summary of running and past Medical problems": What is something like ISSUES, may be it was a NON-ISSUE?

I think "Issues" is OK.  There is clearly regional variation in what this called.  In the US we tend to call these Chronic Medical Problems though usage would seem to indicate "Acute Medical Problems"  would apply in the "New Encounter" or "Chief Complaint".

I don’t think that Medications, Allergies should be included in the “New Encounter” list of “Issues” though it is mandatory for them to be in the “Problem List”.  I think most surgeons would like to see “Surgeries” in this list during the initiation of a “New Encounter”.

How about elements (elements of the history) I think my notion of issues connotes reasons for the encounter being considered at the time generated. Is this the case for other would be complainers?
In point of fact I found this thread when searching “issues” in hope of clarifying what may be evident to the many. I mention this to augment Rod Roark’s warrant in address the term’s meaning…This is distinct from actually COMPLAINING about a great baby in other wise fair bath water.
