Strange Dialogbox appears all of sudden on patient summary page

Hello,

The following dialog box appears all of sudden in between the demographic flow, but its not appearing all the time so I am unable to spot the bug. It appears after closing the actual dialog boxes that shows in page flow. Is anybody have ever seen this kind of error ? Please help me fix this.

Hi @deldev,

Just confirming - are we looking at a test patient or a real patient?

The error is probably in whatever page the dialog.js file is attempting to load, not the javsacript itself.
