Hello,

I was reviewing videos by ZH Healthcare on creating an encounter. During the video the presenter showed their custom SOAP New form. It is not part of the pool of shared custom forms. Is it possible to obtain a copy to customize? I like how they used Nation Notes and do not find it as user friendly as part of the layout based forms. Maybe I’m doing something wrong. I, also, wanted to add diagnoses similar to how they did it but do not know where to begin.

No, it’s not. They were showing how to document an encounter using a SOAP Note constructed from Nation Notes. Their version of Nation Notes had a diagnosis button that opened up an ICD code list you could select from. It’s demonstrated in the 2nd half of the video (https://youtu.be/5Eo1q1mPM5M).

If you are a developer, you can find that the dialog box you are looking for can be found in several places in the code base. I would suggest looking for that javascript to begin your journey.

What are you trying to achive in the SOAP note?

I am not a developer. I do have access to the OpenEMR github, but do not know how to find files and such. I haven’t had time to learn that part as I have been working on learning about contributed forms and how to customize them as well as work with other features. There is so much to learn and so little time. This is my doctoral project and I am trying to demonstrate how OpenEMR can be customized for use at Mason’s free clinic. I’m able to create the Nation Notes, but what I’m interested in is including the Diagnoses into the SOAP similar to how ZH Healthcare did. They have a “Diagnosis” button that causes a pop-up window to show ICD codes for the provider to choose to document the Assessment part of the SOAP. I really liked that feature and how they display the output on the summary page. The way version 4.2.2 works, it does not include the button and the output is displayed in column form. I did see a tip on how to fix that, but I haven’t had a chance to try it out either. Is there a way to contact you outside of the forum? I have other questions and don’t want to clog the forum.

However, what you are asking for requires coding. It is not native to the program.
