I followed the below steps for SMS and EMAIL alerts of group theropy
1.Administration | Globals
2.Enabled the Group Theropy , Click on save button
3.In Navigation bar , Group | New
4.Enter the all details and click on the Add group button
5.Next , Group | Groups
6.Click on Created diabetis group
7.Click on the participants Tab
8.Click on Add button
9.Click on the Participant`s name: text box pop up will come ,search the patient those who have complete demographics data like mobile ,work phone and contact ,trusted emails
10.After select that patients , click on Adding a participant button
11.After selecting the patient who have diabetis problem , it shows below like
