Search or Add Patient Display

Hi, Ive been using the EMR live for the past week. Everything is going well. No changes have been made to any configuration that Im aware of BUT now when pressing the patient Search/Add patient tab has suddenly changed to this and is missing several important fields:

Does anyone know how I get back to the original search/add patient display?

@gizmodoc someone has mess-up with LBF for demographics that you have.

How do you fix it? Is there a specific file I can restore from a backup?

I figured it out. Im not sure how but a field was changed in a drop down menu in CONFIGURATION-APPEARANCE

Sorry for the newbie post. But its been resolved. No big issues after all.
