This and few others like physical exam are to record results of patient observations. Multiple variants exist because of someone felt strongly that their specific format was most productive way to record. Unfortunately they are hard to change for non-developers and are not practice specific templates.

The Review of Systems is that part of the History which comes after Past Medical History/Past Surgical History/Social History.
The ROS mentioned happens to be one of 3 forms which has been contributed in the past & is used by Internists.
We have our own ROS geared to Ophthalmology, as part of a Layout Based Visit Form, which is far easier to modify.

They are essentially the same.
Only difference is the choice of radio buttons or check boxes for the selection of answers.
