Restrict user and patient to facility

Can any one suggest me how to bind patient in facility wise, Since there is a option restrict user to facility is present.

there is a field in the demographics data where one can choose the facility of a patient, i don’t know how binding this is though:
get a patient’s dashboard up, go to the demographics section, go to the ‘choices’ tab - a little way down is a field ’ care team (facility) ’ which lets one sets a facility for this patient.

I’ve also tried, but it’s still the same problem, every patient shows up at all facilities. please find the solution

During patient creation you have to add the facility then you can separate the patient facility wise
