Lab result entries are repeating. If there are 6 result elements under an order grouping it will return 6 full sets of results. The column name will repeat 6 times but the result value/unit/range will change in groups.

Chances are you have not assigned unique code to the tests - see an example. If the code is left blank or duplicated, it will result in error during results processing.
If you are going to assign the codes manually, try to follow the codes reported by labs. They follow industry standard codes.

Your suggestion seems to be working, haven’t got all the codes in yet but repeats have stopped on some entries. I am using LOINC codes along with CPT lab procedure codes. 4.1.2 broke most of my code for HIE, have to rework my lab results, I had results without orders working, CDA prescription import is ok, it uses Kevins prescription_import table and is compatible with his allscripts import functions (I use both daily).
