In the “Patient Report” section of a patient chart, there seems to be a bug when building the check list of items as well as the output.
The scenario occurs when two different providers create separate encounters and choose a form like the SOAP form on the same date of service. When you click “Report” and are in the “Patient Report” screen, you will see two SOAP forms with checkboxes associated with one encounter while the other encounter has no forms associated with it.
When you print the report, the last provider that created an encounter will have their name associated with the forms from the other providers for the same date of service. Thus the “Patient Report” is inaccurately associating provider name with forms for encounters created on the same date of service by different providers.
I am using OpenEMR 4.0 but I noticed this also occurred in OpenEMR 3.0.1.
