Patch 5 - Fee Sheet Categories

OK, I’m a little thick here.  I never realized the Service Categories in Admin - Lists gave options in the “Category” dropdown.  All we have used is the “Unassigned”, and billed the same to all.  Then I read the issue which was supposedly addressed with Patch 5.  In the Superbill (Admin - Services in Tree View) I am not allowed to create two of the same CPT, even if they have different service categories.  The categories show up in the Fee Sheet as drop-down groups, which I had previously simply created by adding numbers and group descriptors.  I still have to do that same thing to put services available in the new Service Categories items.  But my pay rate for a 90801, for example, is different for each payer.  I do not see how I can have that defined by the Service Categories, even though that would seem to be the purpose, because ONLY one 90801 can exist, not one for each category defined.  What am I missing?
