My suggestion would be to use one of the many Developers form anywhere on the world for the software implementation, and a local developer for the hardware.
Distance in not an unsurmountable obstacle anymore, as long as the Internet is available, and time difference might work as an extra favorable factor.

For this you need professional programmers with in good depth knowledge of OpenEMR! Not all specialties have production ready forms.
Also the Agenda might need some tweaking.
The interaction between specialties could give some new impulses for Developers.
Just a bit of patience and probably others will join the discussion with more in-depth solutions.
