I’ve had pretty good luck with KnowledgeTree, the EGroupWare suite has some built in document management functions as well, but KnowledgeTree is simpler to use from an end user point of view.

Hi Sam,
We use OpenKM quite often. Has nice Features such as scanning from the desktop (It calls twain from withing the browser) and the office plugins (both MS and OpenOffice) are free. KT charges for the plugins. The KT interface is simpler and easy to use.

In return for small install and configuration effort, OpenKM seems to offer easy replacement of EMR documents fuctionality. Of great interest here is desktop scanner upload, full text search of document contents, version management and workflows. Looks like it may be possible to treat patient notes and emails / Direct messages as documents as well. It also has utilities that would make it easier to migrate existing documents.
Are there any collaborators willing to work on integration related development? This is great choice for practices with 10,000+ documents.

I have a client expressing interest in using Tesseract OCR for improving document functionality within OpenEMR. Searchability is a key interest.
