How can I design a new html form in open emr? What are the important steps to follow? I need to create forms similar to ROS.
I have created one, which is registered and database installed(No errors during this processes). But when I opened the html form, its not visible?
Also, I need to know, how to delete database?

It seems you have created the form similar to ROS under the folder interface/forms/. But the issue is, the form is not displaying. It may be some permission issue to the folder you have created under forms directory.
Please do share your error log here, so that it would be easy to track the exact issue.
And you can delete the table/table entries manually in PHPMyAdmin screen under 'Administration->Other->Database
