Hi Samsalas,
To correct this go to Administration menu > Practice > Insurance Numbers. From there, select the doctor and click Add New. After choosing the Insurance Company, for the “Rendering Provider Number (Type)”, select “TJ Federal Tax Identification Number” and enter the doctor’s Tax ID number into the Group Number field. This will resolve Box 33B.
For Box 25, that is typically the Federal Tax ID (EIN/SSN) of the rendering provider. The only place I could think that type of number would go is 24J. If 24J is the case, in the same Insurance Numbers screen, you’ll want to fill in the Provider Number and Provider Number (Type) fields.
