Notes like section

Hi Guys,

I am new to openEMR and still grabbing it’s functionality.

I would like to have ‘Notes’ like for patients, where i can have 4-5 fields, how can i achieve this ? I have used LBV forms, they are working however that forms display when an encounter is selected. So is there is anything like Module or Plugin which makes similar kind of functionality that is of NOTES and display on the main fronted page of patient section.

Please Advice.

You mean notes as an attribute of the patient and not a visit? If so, you could add or customize fields in demographics, which are also layout-based.

Thanks for you reply, however what i am looking for it that, currently i am using custom LBF form which will open when i add/select an encounter and the layout based menu has this form to enter the values. However i want to have this form same as Notes OR demographics so that i can fill this form multiple times for an single user without Encounter.

You can try to add your Layout Based Visit form to the end of the Demographics form as suggested by Rod. Or you can add it to the History form.

Best to backup, then try on a test copy of OpenEMR (or 4.1.2 Demo) before attempting above on your working copy.

It is still not clear what you are trying to do. But you are better off using ‘Issues’ - create new category as ‘Notes’ - and use built in forms to record notes. Then you will be able to associate those notes with encounters.

He seems to have a LBV form with 4-5 fields that he wishes to incorporate into the Demographics LBV form. Having 4-5 Issues does not seem to fit the bill. Perhaps Issues don’t have the flexibility as a LBV form.

I have never tried CAMOS. Read a lot about it but I knew it would not fit the needs of my practice.

Time is a premium for most medical offices. Although experimentation is good, it is not appropriate in all cases.

There is no way to create a LBF without first creating an encounter. LBF does not have the flexibility that issues do in this regard.

Just added 4 fields to Demographics in the 4.1.1 Demo.

Not pretty but feasible.
