Hello, I am trying to figure out how to add a new non-encounter form for patients. I see the term " non-encounter form" mentioned on the wiki, but when I search I cannot find any information. By non-encounter form, I mean a form that has persistent data across all visits. Screenshot attached for further clarification.
Any help would be appreciated.

There are a couple different ways to do this. Ideal solution depends on the specific type of form you are trying to create (ie. what data will it be holding).

Could this be something that you add to either the Demographics or History layouts in Administration->Layouts
(before you make any changes in the layouts editor, ensure you have backed up your installation)
