I am having an issue with Box 11 on HCFA Form 1500 pulling the information when printing secondary claims. I have verified that the claim is set to go to the secondary payer. However, Box 11 is blank even though the insured group number is in the chart. I did find a possible “fix” and would like to know if this does correct the issue. Louisiana Health Care Connections will not process the claim without the information and I am unable to add the information by filling the box in myself.
We are running OpenEMR version 7.0.3 (Patch 3).
There is a known bug in the library/classes/Hcfa1500.php file where shifting a claim sequence to secondary (-1) clears the primary insurance group data array out of memory. This leaves Box 11 completely blank on paper CMS-1500 secondary claims. Please download and install the latest official cumulative patch for 7.0.3 from the OpenEMR Patches Wiki to resolve this mapping error.
