I have had this problem and looked around with no working solutions. Some sites and thread suggested i use command shell which to me is not an option since i am on a shared hosting plan and dont have acces to a command shell.

So what i did was,1; i deleted my MySQL database using phpAdmin. 2; created a new database and then before poiting openemr to the database for population i first used phpAdmin in operations under “Collation” tab to change that empty database coallation to utf8_general ci 3; went back to openemr installation and chose that i have already created a database and just entered the relevant details. Left the 2 fields to Localhost not an IP.
