Hi, The doctor I am helping with OpenEMR would like to be able to print a procedure order with the address or addresses of the recommended facility to perform the tests (like XRay lab or blood work lab). Is there a way of doing this with OpenEMR? Thanks!

The Referral form with some modifications will fit the bill.
To see what the form looks like, Reports->Blank Forms->Referral.
To modify the form, Administration->Layouts->Referrals.
To generate the form, Patient Summary->Transactions.
Listboxes can be used to populate the referred to facilities and their addresses as well as the reasons for the referral, so that the secretary can just choose from the drop down menus instead of having to manually enter these fields each time.

Are you on 4.1.2? There you would select the service provider for every order. You can then include address in the name - not pretty but it should work.
