Modify "Receipt for Payment" document

I’m looking at having my patients pay me at time of service and then bill insurance directly themselves. Some companies want a HCFA to document the services rendered. Some just want a receipt, but a detailed itemized receipt.

The detailed receipt produced from the fee sheet dialogue in OEMR 5.0.0 does not provide all of the details I need. I’m interested in modifying this receipt form (I attach a copy with my comments on what I’d like to add.) But I have no idea where it lives, or how one would go about modifying it. Is this something that can be altered by adjusting parameters in phpmyadmin, or through some formatting manager within OEMR? Or does it require writing PHP code from scratch (something that is beyond my current understanding and abilities).

I’d like to be able to control and format this myself, but I have no idea how to get started.
