How can I modify the insurance form used to create / manage patient details?
You would need to go to the /interface/patient_file/summary/demographics_full.php. This part of the page is hard coded. It is not a LBF.

What are you trying to address?
There are no plans that I know of to change the make up of the insurance section of the patient dashboard.
We use different definitions / terms here for insurance, and I thought by using the Language Definitions / Constants translation tool I would address that. Another thing is that I do not need all the fields, how can I omit or hide them?
To hid or change the fields of the insurance section, it has to be coded. The insurance section is not an editable form from the LBF interface. You can post a message on the job board. There are developers that would take on the work for you.
