Hi folks: Can someone think of a reason why the Misc Billing Options form would not show?
I have confirmed the link it correct because if I replace forms/misc_billing_options/new.php with a simple HELLO, it shows up. All other forms (such as the fee sheet) show up fine but when I select the Misc Billing Options, I get a blank screen (with the tabs on top). I have tried on with several different patients/encounters?

The 5th line of new.php for the misc_billing_options/new.php file is:
require_once(“date_qualifier_options.php”);
I cannot find this file anywhere in the tree. Commenting this line out makes the form display… I’m just afraid something will be missing :)…

Ahhhh… yes… of course…
the main reason I wanted to open this was to set Box 14 (Date Qualifier). Go figure.

Sorry to bother you all… I coped the misc_billing_options from a newly downloaded source tree and added it to my tree and it worked. Not sure why it was missing in the first place.
