Medication Reconciliation Performed

There is this anesthesiologist that is using the EMR and had a question about Medication Reconciliation Performed box can only be checked after checking the Transistion of Care box first. They don’t do transition of Care. So is there a way to move that from under transistion of care without coding?

Is there another place to do Medication Reconciliation?

“Medication Reconciliation Performed” is shorthand for a process that occurs after a physician accepts a patient recently transferred into his practice.

The first box is checked when this new patient has been accepted.

The second box is checked when the physician has reviewed the medical record for said patient.

Both boxes are used to calculate the numerators/denominators for this MU measure.

Anesthesiology has to keep track of the meds they use, especially narcotics. If this is the reason for the request, the accounting can be done as another Issue or as a NationNote as part of an encounter.

Either approach can work; because, unless the patient has a complication requiring a return to the OR, there is generally only 1 encounter for any given period of time. This accounting is usually not a recurring task.

Another possibility is to use the Prescription module.

Additionally, Anesthesiology is granted automatic relief from penalties levied for non-attestation of MU.

As a result there is no reason for them to fool around with the boxes mentioned above.

The same is true for Diagnostic & Interventional Radiology.

If an EP is classified in the Medicare Provider Enrollment, Chain and Ownership System (PECOS) as having one of the following 5 specialty codes as the primary area of practice, the EP does not need to submit this form. The EP is automatically granted a hardship exception from the 2017 payment adjustment based on the designation in PECOS.
Anesthesiology (05)
Diagnostic Radiology (30)
Interventional Radiology (94)
Nuclear Medicine (36)
Pathology (22)

PECOS Specialty Codes.

Most Anesthesiologists would have applied for their NPI under specialty code 05 rather than 72.
