Thanks for your support. I managed to fix the issue simply by repairing my database. I am using ver 4.1.2 on windows platform with wamp server 2.2.
I have installed the latest ver 5 of openemr with latest wamp server on my personal computer for testing. I thinks its awesome.
I need some assistance and guidance to upgrade my system to new version without losing any data.
Your help will be highly appreciated.
