I created a small form for the common cases in which we need to swab a patient for PCR virus detection. We need to ask him how he feels, who is the possible close contact that transmitted the virus to the patient, and how many work and family contacts are likely to acquire the virus. If anyone wants it, I can export content for 2 tables in the MySQL database. You can see the form in my website. I am ready to add more questions that help define the infected contacts.
