Is it possible please to put a comprehensive video on how to make those forms in a systematic way , especially how to make the fields dynamic where it can pull from other tables? I have seen one done by Samantha and the moderators on the forum but it was allover the place and I got lost in it …. please do one and link it here as a reply so we learn how to modify our intake/PE forms because the cookie cutter ones are short of what a daily practice needs.
Hello @Mohammad
I sympathize with your frustration about poor documentation, it is the curse of FOSS projects.
You are right about the cookie cutter forms, but one thing about a comprehensive LBF tutorial is that it has so many options and variables it would be difficult to show them all. And then a proper tutorial would need to show how to build a basic form and demonstrate what the options did, so the user could build the form they need.
I presume you have read through the OpenEMR wiki’s collection of documentation on Layout Based Forms that can be found with this search?
https://www.open-emr.org/wiki/index.php?search=layout+based&title=Special%3ASearch&profile=default&fulltext=1
But also, can you post the link to the video you watched that was so disorganized? I’d like to take a look at it.

Hello Team
We have done a LBF detailed tutorial Video - Please check the below link.
Hope this will be helpful.
Comprehensive Guide For Creating and Using LBF Forms in OpenEMR
If any other details required in LBF- let us know. We will try to cover that as part 2 tutorial.

Thank you for the link , I am dizzy…!
there is a need with screen shots if not video to explain the myriad options for the fields in a systematic way instead of trying to find what each thing does by try and error.
or
if any GUI that plugs in and can do what I have always done in Filemaker Pro which amazingly combines the layout making with tables and fields, here I am lost , I need something for dummies and consider me one . I need to make forms that are meaningful , suitable to the intent of the practice and equally appealing to phone screen , I pad screen and obviously to computer screen real estate.
Hi @Mohammad
The wiki does have a fair amount of info on LBF/ LBVs (same thing, different abbreviations).
@gutiersa has written a most excellent doc on the topic, and the link takes you straight to the definitions of the LBF Options. But read the rest of the page for even more good info.
https://www.open-emr.org/wiki/index.php/LBV_Forms#Options
Search the wiki for everything ‘layout based’ with this:

I read it … just adding to the confusion, for example what is this suppose to mean?
Hints for help on laying out fields:
- Keep first object as Label Col 1 and Data Col 6
Keep the rest all in the same row as 0 and 0
This will start the new line. - Keep last object on the row as Label Col 0 and Data Col 6
Add a new field with Label as any text or even a period, Data Type as Static Text, Label Col as 1, and Data Col as 6 leaving comment blank.
Save new field. Then delete the Label and save the changes.
This will insert a blank line.
more confusion.
This a slow video but a very useful video. Grab your cup of coffee and notebook.
I watches it many times and it gave me confidence to create lbf forms in different forms.
One of the best videos.
which video I do not see link in your post?
