Flummoxed by cds rule logic

Thank you for your reply. Maybe the first step would be to be full-compliance with smart-on-FHIR, I’m thinking in particular of SMART Backend Services (http://build.fhir.org/ig/HL7/bulk-data-export/authorization/index.html).
