I want to display phone numbers here .
how do i do it ?

These two fields are found in the Communication Engine of the Eye Form. They draw their data from the address book. The first list builds from providers only - providers are identified by the presence of an NPI number. The second includes everyone - referrals can come from friends and family too so if you want to track other referral sources, add all their data into the address book. If you include phone numbers and fax numbers, they will appear here in the Communication Engine along with the address info. You can fax the complete report to either PCP/referrer by clicking the fax icon (which appears when a fax number is present). I do not believe faxing is setup in the codebase, because faxing is site specific (hylafax, email/fax gateway, API call, etc). If I recall clicking the icon will build the PDF report, store it and send it via email to an email fax server. You have to get into the code to complete this setup for your practice.

I don’t know if it applies in India, but if you install ICD-10 and CPT4 codes, based what is documented, it auto-populates the Impression Plan options, suggests billing codes, and subsequently completes the Fee Sheet.
