I want to create a LBF form and want it to appear in clinical - here

While creating the LBF form - you have to mention the Category as Clinical (as shown in image below) to appear in Clinical menu.

i tried to have the pt report value and the normal value in one row, but after saving all spaces between them vanish.

i want the saved report to look like this.

Hello Fernando

You can edit demographics in Layouts - you can add new sections / remove sections, add/ remove fields inside the sections, etc…

Similar to Layout based Forms (which will appear under Encounter) you can also create Layout Based Transactions (which will appear under Transactions in patient dashboard).
