How to print with digital signatures and patient name and DOS on each page?

  2. New Site: OpenEMR v8.0 (PHP 8.3)(Cloned database from v6.0 upgraded via sql_upgrade.php to v8.0)
  3. Environment: Linux / cPanel

Our clinicians E-sign their progress notes which are layout based forms. When we go into the patient dashboard, then click report, and select the progress notes and encounters to print, they will print with the signature, but they will not print with the patient name and date of service on each page.
When we go into each encounter individually and print. They will print with name and date of service on each page but it will not print the digital signature.
Is there a setting or way to print a report or save pdf so progress notes or encounters will show the patient name and date of on each page and show the clinicians digital signature?

We have an audit where we have to provide all documents from a certain time period and each note must show signature, and name and date of service on each page.

As of last night I’ve tried to update our site from 6.0 patch 3 to 8.0 to see if it could work. I copied our database and moved the site files to the new install. On 8 the patient report is printing name and date of birth but not date of service on each page and it still was not showing signature. Currently I have both versions installed so if there’s a fix for either that would be very helpful.
