How to point it to a new "listbox with add " like we can in admin>demographics

You really have to give us more information than what you have just presented. A close-up of a form does not show us where the form is in the system. You did not share what I am reading where this form is located. It does not look like the demographics form that is native to the system. And there is no Admin > Demographics in the config. So, that leaves me trying to guess where you are referring to. But if I had to make a guess that you are asking about an LBF. Then simply change the datatype. If you are referring to a hardcoded form. Then you will have to summons the drop-down with add from the options include.

There s no demographics in admin >config.
The form i posted a snapshot of is showing inhouse inventry, etc, so i thought it would be evident to a person who has been using openemr for sometime.
anyway it is from inventry>management ( top row, right of modules.
Because it is inventory management. That is hard coded. You will need to get into the codebase and options include to generate that dropdown.
