Hi Sourav,

In Administration->Layouts menu, select Demographics and click on ‘Add Field’ next to the group where you would like to add a button. Select the datatype as ‘Checkbox/Radiobutton’ and enter all other required details and click on ‘Save New Field’.

Note: Clicking on List field will allow you to select from already available Active list. You can also add a list of your choice by clicking on Administration -> Lists menu and click on New List button.
