I tried to import hcpcs codes downloaded from ( https://www.open-emr.org/wiki/index.php/Code_Types#Via_.csv_Import )
I logged in to the database of openemr 5.01(6) and clicked import in the codes table.
The attachment was the error generated.
How can this be fixed.

Have you checked the codes table? You may already have value with id 147.
Now you can remove value with id(147) in codes tables and import the file.
Another option, you can change the id(max id in codes table + 1 ) in import file.
