HCFA 1500 Printing

I remember someone said that the HCFA 1500’s were printing with a missing decimal point in the charge amounts, but cannot find the referece.  I also thought someone said that was fixed.

However it still seems to be broken in CVS.  Am I mistaken?  Did the fix not get checked in?

Also the copay is appearing as a negative amount, and is therefore resulting a balance due greater than total charges.  Has nobody noticed that?

Item 11 Insured’s  Policy Group or FECA number is printing out the Insurance company name but it is correct in the EDI file.
Also item 25 Federal Tax ID number:  I havent been able to get it to print the SS Number and mark SSN instead of EIN.  Looking at hcfa.pl hcfapdf.pl it looks like only EIN and Federal are hardcoded in.
