To clarify. Are you asking how to force the external id to be auto generated (and not allow manual entry of this item). If so, I think this would require minimal php coding to the demographics form php script.

openemr/interface/new/new.php displays a new user form
Layout is at Administration->Layouts->Demographics
(the layout stuff is stored in the layout_options mysql table)
