Form issue with Patient Portal

I am encountering an error when trying to upload forms to the patient portal. When uploading an editable pdf, I receive the error of “either pid of file id has an invisible text field that is missing from uploaded pdf.”

Is this a known issue or am I doing something wrong? Thanks!

Use Adobe Life Cycle designer ES2, with:
a. 2 invisible text fields; pid and fileid.
b. HTTP submit button.
c. and some additional fields; plname, pdob.
d.local save on (enables the posibility for clients who are using the form in the offsite portal to upload their pdf doc.)

Is there something I have to add or am I doing something wrong?

In the week of May 17th 2016 the form upload problem will be solved by ZHHealtcare (thx again for the great support). It was a pdf library issue.

For users wanted to use the upload pdf-form in the offsite portal …
The directions mentioned in the link by BRadd must be used in the pdf. For the “local save” option use an Adobe Acrobat pro version, under “save as”.

Note: the local save option gives a client the needed permission to fill in a pdf form and to make it possible to upload the pdf-form in the portal.
