Eye Exam Screen doesn't show all components and Spins

Situation
Eye Exam Screen doesn’t show all components and Spins. This is the first I am installing this openemr.

OpenEMR Version
I’m using OpenEMR openemr-7.0.1

Browser:
I’m using: Chrome

Operating System
I’m using: Linux junctioncity 4.14.93-grsec-grsec+

Search
Did you search the forum for similar questions? Yes and didn’t find anything

Logs
Did you check the logs? - Yes
Was there anything pertinent in them?
Please paste them here (surround with three backticks (```) for readability.
You can also turn on User Debugging under Administration->Globals->Logging User Debugging Options=>All → Don’t see Admin->Globals option and i see Admin->System->Logs and don’t see anything.

Seeing this on the browser console

@stephenwaite Nice, after i applied the first patch i can see all the components. Thanks for your help.

Also one another thing I am noticing on my & demo instance that a warning appears “Warning: Read only mode” and I can’t save anything only on PMSFH box. Any suggestions?. Thanks.
