If we focus on the forms (and not encounter level signing), then it seems pretty intuitive. Create a form, save it, then sign it. After sign it, can no longer edit it, but can add an addendum to the end of a signed form and sign that. Is there an issue with that workflow?
Personally, I think every saved and signed note revision should be saved for history. Personally, I think should be able to modify/edit a signed note as long as sign it again (and the revision history of this is saved).
