Errors on X12, submission to Medicaid

Hello,

I’ve got to submit 837P Claims to NC Medicaid and get the following errors on a validation tool. Using 5.02 dev version and filled out the x12 parameters under Admin/Practice/X12 per the specifications guide. I understand there is some previous discussion on X12 topics but the posts are so long not sure if/where corrected code has been submitted. Note, this is all test data so no worry about PHI. Whoever can help, greatly appreciated. Thanks!
