What version of OpenEMR are you using?
What platform?
What is the exact link you clicked before receiving this error?

Windows, running on Xamp server. Its a 5.0.1 of OpenEMR/

@Robert, I figure out the error, it was the s.s field that caused the JSON error. Fixed now.
