You have to assure the proper Service Facility has been selected in the Encounter, and that beforehand its NPI and EIN have both been properly defined in Admin - Users - Facility. The EIN for the Service Facility should carry to CMS 1500 field 25, and the Facility & NPI should carry to 32 & 32a, while the Biller info will be in 33 & 33a. Please let us know if that is NOT the case.

My Bad! I now see that the Biller EIN is what is in field 25. Now I am confused. CMS says the TIN to use there is the TIN for the individual provider or the facility who will pay that individual provider whose NPI is shown in 24J. That is why medicare insists that a sole proprietorship must have the actual proprietor, who must be the provider, as the legal name in their records, NOT the DBA name they use. So my wife’s practice is shown in NGS records as Lynne Odell-Holzer DBA Comp Holistic Health & House Calls, which is exactly how IRS shows her EIN.
The posting point is from near line 494 in …/library/gen_hcfa_1500.inc.php labeled as // 25. Federal Tax ID. That we happen to be doing our own billing means our TIN is properly posted. But if your biller is different, that might be a problem, as CMS will ONLY pay the providers themselves. Sorry, however you decide to fix that.
