Does anyone use/know what all the different options for the Layout Based Forms (LBF) do?

Hello All!

I’m wondering if anyone has successfully used all – or most – of the different types in the layout based forms?

The wiki pages are a bit vague and the information on the forums are either outdated or incomplete.

I am referring specifically to this:

And extra points if you know how those types relate/work with the LBF options!!

Most of them have been reviewed by the amazing @gutiersa in her fabulous youtube video, thank you so much!

However, there are others that are haunting me because I cannot figure out how to use them. Curiosity is a blessing and curse lol

List box & List box w/add
I use List box w/add to build a list of referring physicians in place of the one already there so it does not send to insurance using the CMS-1500 form. Then I add the physicans to it from the demographic sections

I created a list called ref physicians and I selected it in the LBF.

Do you send back the initial encounter note as well based off of that list? We currently use the ref provider that is built into OEMR because it ties to the address book, but we used to do it your way in the past.

We collect the referring provider name, phone, fax numbers and a small note in the patient demography. When we send the note back we copy and paste the name , fax and note to the efax .

I know one time we had issue with the refering provider section and we had to remove it for teh claimto go through and be fixed. Since then we do not use the builed in referring provider section.

I wonder if that is something that can be turned off in the Admin > Config > Encounter Form

Additional curiosity – I wonder if this was build in for medicare services?

Anyone else know anything about other other options?

These are the interesting ones I haven’t been able to figure out fully:

  • Address list
  • Billing codes ← This is something I’m really trying to figure out where the logic ties into
  • Exam Results
  • Image canvas
  • Insurances
  • Lab results
  • Organizations
  • Visit categories

I’ve narrowed down that the source for these items makes a difference, i.e. Patient, Visit, Form, History, and VisForm.

It’s been a few years since I did a deep dive into any of these as I’ve been working on integrating FHIR more and more, but I’d like to know if anyone else has discovered anything that is practical for a physicians’ encounter notes.
