Distortion in openemr php code

Good day, I am new to openemr. Just installed and i seem to have issues with php script distortion which is present in all the openermr tabs. I attached screenshots for clarity.

Undefined variable: group_fields after install

Please anyone can help me with the problem. I replaced this setting in the php.ini line 445 to line 440 with error_reporting starting the line and leave out (show only errors),if you are using notepad++
